Socially Determined seeks to understand actionable measures across broad populations, by defining extensible measures of risk down to a 200 m radius across the entire country. Additionally, they have created measures of social risk that they can create for individuals in a population. In this talk, Alan will focus on how those measures can help improve outcomes for a typical Medicare Advantage population. By clustering on risk patterns and then applying machine learning, they are able to predict those individuals most likely to fall short of desired preventative care goals. In this way, an organization can direct its limited resources towards those most in need of help.
